REGISTRATION INFORMATION

Please include your completed 2010 Registration Form and Medical Information form with your payment. A medical form should be completed with the registration form if your child has NOT attended camp in 2010 prior to the session you are registering for.

Week long camps (at Ted Reeve): Payments are cash or cheque payable to ‘Hockey Fundamentals Camp Inc.’ No post-dated cheques accepted. Please see below for early registration and multiple registration discounts.

Saturday classes (at Beaches Sports Centre): Payments are cash or cheque payable to ‘Beaches Sports Centre.’ Sorry no early registration or multiple registration discounts.

Summer Camp 2010 Discount Program:

Early-bird:	Payments made befire July 1 save the 8% portion of the H.S.T

Family:	When 2 or more family members are attending camp, you'll save an additional $10 per child per week

Multiple Week:	If registering for more than one week of camp, you save an additional $10 per child per week

Loyalty Program:	Deduct $20 per child per week if your child was registered for any 2009 or 2010 program, including Saturday programs
