REGISTRATION INFORMATION

Please include your completed 2007 Registration Form and Medical Information form with your payment. A medical form should be completed with the registration form if your child has NOT attended camp in 2007 prior to the session you are registering for.

Week long camps (at Ted Reeve): Payments are cash or cheque payable to ‘Hockey Fundamentals Camp.’ Fee includes G.S.T. and camp t-shirts . No post-dated cheques accepted. Please see below for early registration and multiple registration discounts.

Saturday classes (at Beaches Sports Centre): Payments are cash or cheque payable to ‘Beaches Sports Centre.’ Sorry no early registration or multiple registration discounts.

Discount Information:

Christmas Camp:

$20.00 off per child if full payment received by December 1, 2007. In addition deduct $10.00 off per child if more than one child registered (regardless of payment date)

March Break Camp:

Up to $60.00 off per child if full payment received by February 10, 2008.

NEW – LOYALTY PROGRAM – If your child was at camp for any week in 2007 or the 2007/08 Christmas camp deduct $20.00 per child.

Early Registration: If your payment is received by February 10^th, 2008 deduct $30.00 per child.

Family discount: If more than one child in your family registers for camp deduct $10.00 per child.

Summer Camp:

$30.00 off per child if full payment received by May 20, 2008. In addition deduct $10.00 off per child if more than one child registered (regardless of payment date)

Camp T-Shirts:

Camp T-Shirts are free for participants in full-day camps. Saturday skating participants or hourly camp participants can purchase camp T-shirts for $10.00 (Youth & Adult sizes available).
